Ages and Stages Questionnaires (ASQ-3 and ASQ:SE-2)
ASQ is an assessment tool that helps parents learn about the developmental status of their young children across five developmental areas: communication, gross motor, fine motor, problem solving, and personal-social. The assessment tool is available in 21 versions to allow measurement of development at the following months: 2, 4, 6, 8, 9, 10, 12, 14, 16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months of age. There is an age calculator to assist in choosing the right tool by factoring in weeks born premature. The availability of the tool at various age levels allows the developmental progress of the children to be tracked over time.
There are two questionnaires - the ASQ- 3 questionnaire looks at your child's general development. The results help determine if a child's development is on schedule. The ASQ:SE-2 questionnaire looks more closely at your child’s social emotional development. Results help identify appropriate social emotional competence in young children.
What do I need to do?
- Let us know you would like to participate! We can send you an electronic link to the questionnaire, or give you a hard copy if you'd prefer.
- Review and accept the information on the parent consent form.
- Answer a few questions about your child and family.
- Fill out the age appropriate ASQ-3 and ASQ:SE-2 questionnaires for your child.
What happens after I complete the questionnaire?
- Ms. Emily will share the results with you. Ideally in person, however results can be discussed via telephone and sent to you by email or mail at your request.
- We will share a book and fun play activities that will foster your child's development.
